Implanter un dispositif de biologie délocalisé dans les services de soins est un défi. Particulièrement si celui-ci permet le dosage hypersensible le la troponine et ainsi l’application de l’algorithme rapide de stratification du risque dans le cadre de la prise en charge des patients suspects de SCA aux urgences. Cet article, proposé par le groupe d’experts de l’International Federation of Clinical Chemistry and Laboratory Medicine Committee on Clinical Application of Cardiac Bio-Markers (IFCC C-CB), propose des recommandations et un cadre pratique d’implantation, sur tous ses aspects (validation et vérification des procédures, responsabilité des fournisseurs, vérifications sur site avant utilisation en routine). Un document essentiel pour tous les professionnels impliqués dans cette démarche (fournisseurs, biologistes, urgentistes), et pour garantir le succès de ce changement de pratique.

Pourquoi une telle enquête ? Parce qu’il semblait essentiel de mesurer au niveau national les délais de rendu des résultats de troponine par les laboratoires hospitaliers (27 centres participants), dans le cadre de la prise en charge des patients suspects de SCA aux urgences : non seulement mesurer le délai global (81 minutes), mais aussi montrer la part du temps d’acheminement de l’échantillon au laboratoire (19 minutes, soit 23% du temps global). Ce délai de rendu, souvent utilisé comme indicateur pour les laboratoires, est plus rapide lorsque le site bénéficie d’une prescription connectée ou d’un système expert de validation biologique. L’objectif d’un délai de rendu <1h pourrait être atteint dans une situation de biologie délocalisée (pas d’acheminement de l’échantillon au laboratoire, pas de centrifugation, obtention du résultat avant validation biologique).
